Does the progress note extractor read the SOAP structure?

Accepted Answer

Yes. The subjective (patient-reported symptoms and chief complaint), objective (observations and exam findings), assessment (clinical impression), and plan (treatment and follow-up) come back as their own fields, alongside the conclusion (impression). The tool reports these as written and does not interpret them.

Which patient, provider, and encounter fields come out?

Accepted Answer

The document number and date, the patient name, medical record number, date of birth, and gender, the documenting provider name and credentials, the encounter reference, the note status (draft, final, or amended), and the managing organization that holds the record.

Are vital signs captured?

Accepted Answer

Yes, both as fields and as a table. The temperature, blood pressure, heart rate, and respiratory rate come back as fields, and the vital signs table returns each vital type, value, and measurement time.

Question 4

How are medications, diagnoses, and observations returned?

Accepted Answer

The medications table returns each drug with its dosage and status (plus the free-text dosing instructions), the diagnoses table returns each ICD-10 code with its description, the observations table returns each observation code (typically LOINC) with its value and date, and a procedures table returns each procedure name and date. The field set follows HL7 FHIR R4 resources with LOINC codes.

Is it safe to upload a note that contains patient data?

Accepted Answer

The note is processed via the Talonic API for extraction only, is not retained for training, and is not shared. Handle identified PHI with care and redact where you can; for BAA-covered work, ask about the Talonic platform. Uploads are PDF only, up to 10MB and 100 pages.
