The notice number and date, the policy number, the coverage type (enum), the insured name and address, the insurer name and NAIC number, the policy effective and expiration dates, and the producer (agent or broker) name and contact.
A cancellation notice ends a policy before its term is up (or declines its renewal): it states the reason, the effective cancellation date, and the notice period. An endorsement changes a policy mid-term without ending it, and a renewal continues it into a new term. For those use the policy endorsement or policy renewal extractors.
