The selected health plan, dental plan, and vision plan each come back as fields (by name or code), and an elected benefits table returns each benefit type with its plan name, coverage tier, and effective and expiration dates, so the full set of elections is captured.

Are beneficiaries and coverage details returned as tables?

Accepted Answer

Yes. The beneficiaries table returns each beneficiary name, relationship, and date of birth, and a coverage details table returns each coverage type with its plan code, monthly contribution, employer contribution, deductible, and covered dependents, so contributions reconcile per plan.

Does it record the acknowledgment and signature?

Accepted Answer

Whether the employee acknowledged reading the plan documents and waivers comes back as a boolean, and the employee signature (or digital authorization), the employment start date, and the employee address (city, region, postal code, and country) are read too.

The form is processed via the Talonic API for extraction only, is not retained for training, and is not shared. Redact dates of birth and other personal details where you can. PDF only, up to 10MB and 100 pages.
