The log or form number, the establishment name and address, the employer identification number (EIN), the calendar year logged, and the log period start and end dates (typically January 1 to December 31 of the reporting year).
Per entry: the case number, the employee name and job title, the department, the incident date and date reported, the injury description, the body part affected, the type of injury (enum), and whether the case is work-related and OSHA recordable.
